Position Summary

Serves as social worker to the Vulnerable Elder Protection Team \(VEPT\), a first\-of\-its\-kind Emergency Department\-based consult service focused on optimizing care for potential victims of elder abuse\. Supports program operations, assists with clinical management of cases and participates in community outreach\.

Job Responsibilities

+ Triages coordination between VEPT clinical team and Emergency Department Social Worker who may provide initial assessment and front\-line care for patients evaluated by the VEPT\.

+ Interacts clinically with patients\. Provides individuals and families with psychosocial support to cope with elder abuse\. Advises family caregivers, provides patient education and supportive counseling and makes referrals to community resources\.

+ May conduct psychosocial assessment upon patient arrival\.

+ Assists with design, refinement and maintenance of VEPT clinical protocols based on clinical experience\.

+ Follows\-up longitudinally on care, on medical, psychosocial and legal outcomes of VEPT patients using chart review and contacts within the elder services community\.

+ Assists with coordination with New York City Elder Abuse Center \(NYCEAC\), the NYC Department for the Aging, and Adult Protective Services to integrate the VEPT with existing programs\.

+ Liaises between geriatrics, patient services, ethics, and other disciplines to coordinate care for VEPT patients\.

+ Facilitates communication between the NYP Care team and outpatient or community\-based agencies, organizations, and providers\.

+ Advises community agencies on potential VEPT referrals, including the coordination of transportation of VEPT referrals/activations and preparing the ED and team for arrival\.

+ Attends Multi\-Disciplinary Team \(MDT\) meetings in all NYC boroughs, serving as a representative of VEPT and presenting cases as appropriate\.


+ Master's Degree


Experience with facilitating inter\-disciplinary groups\.

Knowledge of elder abuse, aging and/or NYC services for adults preferred\.

Knowledge, Skills and Abilities

+ Ability to work collaboratively with internal and external constituents\.

+ Demonstrated organizational skills and ability to pay close attention to detail\.

+ Advanced level of communication and interpersonal skills\.

+ Patience and a high capacity for empathy and perseverance\.

Licenses and Certifications

+ Licensed Master Social Worker \(LMSW\)

Working Conditions/Physical Demands

Ability to work flexible hours as needed\.

Ability to travel throughout NYC boroughs for meetings as needed\.
