Description Position Summary
Working with the leadership of the division of Population Health, the Project Manager will collaborate with Weill Cornell Medicine's Primary Care and Specialty departments to create and implement strategies for overall quality improvement and specifically closure of clinical and preventive gaps in care. The project manager will use principle of Patient Centered Medical Home to further the triple aim of improved quality, reduction in total cost of care and increased patient satisfaction.
Job Responsibilities
+ Serve as a primary liaison between the division of Population Health, Managed Care, POIS and Weill Cornell Medicine practices, to further the Patient Centered Medical Home Mission.

+ Use data to assess trends for individual practices. Identify areas of improvement for clinical and preventive quality measures.

+ Disseminate data on a regular basis to clinical and administrative staff at various practices on key quality measures.

+ Collaborate with individual practices to identify challenges and opportunities, and create a strategic plan to facilitate gap closure.

+ Work with administrative and clinical leadership to identify training opportunities for practice staff, to improve overall patient care and support PCMH efforts.

+ Identify process improvement opportunities for individual practices and for the Weill Cornell Medicine system as a whole.

+ Develop and implement monitoring tools to measure outcomes on an ongoing basis

+ Prepare presentations on project activities and outcomes for leadership meetings

+ Assists with special projects as needed.
+ Bachelor's Degree in related field
5. years in project management.
Prior experience in a health care setting.
Knowledge of Patient Centered Medical Home, Practice Transformation, ACO and Quality Improvement in a Health Care Environment
Knowledge, Skills and Abilities
+ Familiarity with Microsoft Excel, PowerPoint, Publisher and Endnote is highly desired.

+ Clinical background or knowledge

+ Knowledge of SQL and other software applications.
