Position Summary







This role reduces the incidence of unreviewed and uncommunicated diagnostic test results in the ambulatory setting via implementation of the funded project entitled Tracking and Communication of Diagnostic Test Results to Patients\. A large share of malpractice costs and patient grievances are related to delayed review and communication of test results and follow\-up plans to patients\. The goal of the project is to reduce the incidence of these occurrences in order to decrease the likelihood of delayed treatment and improve quality and patient satisfaction\.







Job Responsibilities







+ Disseminate guidelines for reviewing and communicating diagnostic test results to patients\.



+ Standardize provider documentation of review and communication\.



+ Measure baseline performance on diagnostic test completion, review, and communication\. Collaborates internally to generate reports from Epic\.



+ Generate process maps at the practice level, including detailed workflows around specific follow\-up tasks and ownership of those tasks\. Assigns responsibility as appropriate\.



+ Through automated reports and manual review of charts, generate reports at the practice and provider levels with metrics on diagnostic tests\.



+ Report metrics to providers, such as frequency of unresulted tests and documentation of review and communication of results to patients\.



+ Build relationships with physicians and practice managers\.



+ Identifies opportunities for process redesign, leads small tests of change, and follows metrics in real time for evidence of improvement\. Sustains successful changes\.







Education







+ Bachelor's Degree







Experience







Masters Degree or Project Management Professional \(PMP\) Certification preferred







5\+ years of increasing levels of experience and responsibility in healthcare\.







2\+ years of experience with quality and performance improvement\.\.







Ability to work with and effectively communicate with clinical staff and physicians\.







Strong organizational and planning skills\.







Proficient with Microsoft Word, Excel and PowerPoint applications\.







Knowledge, Skills and Abilities







+ Previous experience with data mining and data analytics\.



+ Demonstrated excellent verbal and written communications, and interpersonal skills\.



+ Strong background in developing trust and confidence in relationships with cross\-functional teams\.



+ Demonstrated presentation skills\.
