Position Summary

Under direction, assists with and maintains community outreach work for veterans and adolescents with substance abuse issues

Job Responsibilities

+ Arranges meetings and recruits speakers to inform the community about program goals\.

+ Distributes informational material to residents of specific municipalities, school districts and/or geographic areas to describe what, where, and when program services are available to them\.

+ Tracks work progress within spreadsheets, word processing, calendars, e\-mail and database software\.

+ Prepares monthly reports on services provided by gathering community data to assess program effectiveness\.

+ Helps clients engage in the recovery process and become a part of their community by encouraging personal growth, assisting in recreational activities, reinforcing healthy coping skills and facilitating positive socialization and reintegration\.

+ Assists in providing liaison services between clients, potential clients and other social agencies providing services for client population\.

+ Assists in scheduling appointments for clients to meet with program professionals\.

+ Establishes and maintains contact with families of program participants where applicable\.

+ Assists program staff with follow\-up on cases\.

+ Participates in staff meetings and consultations to receive and provide input on status of patients\.


+ High School Diploma


Previous outreach experience with vulnerable populations

Familiar with diverse New York communities

Basic knowledge of breast health awareness and disparities

Breast health/Breast Education experience preferred

Facilitate community\-based education events
