Position Summary







Working under the supervision of the Program leadership, supports the Population Health Division in data collection, analysis, reporting and presentations, as it relates to commercial and Medicare ACO contractual agreements\.







Job Responsibilities







+ Integrate data from multiple internal and external sources, such as EMR and Payer data, including but not limited to claims and clinical data



+ Analyze data to identify patterns, attribution changes, trends and discrepancies\. Create reports and presentations based on the above\.



+ Create regular reports and presentations for internal review by leadership



+ Provide reports to practices on gaps in care and chronic conditions as directed by Population Health Leadership, and as requested by individual practice leadership



+ Under the supervision of Population Health leadership, assist in chart reviews to identify missing clinical and preventive gaps



+ Work with POIS to generate internal data requests and analyze raw data to create needed reports



+ Work with Population Health Leadership and fellow analysts to assist in using Payer Portals to generate daily reports on transitions of care for Care Managers to use for calling hospitalized patients



+ Work on generating reports using multiple data sources to identify high risk patients in need of individual care management



+ Keep a running inventory of all payer reports as well internal reports and track changes on a regular basis



+ Coordinate with external payers and POIS to ensure timely receipt of reports







Education







+ Bachelor's Degree in related field







Experience







Minimum 3\-4 yrs prior analytical experience







Experience as a Healthcare Data Analyst in a Payer or Provider setting highly desirable







Bachelors or Masters Degree in relevant sciences, Health Informatics, Statistics, Mathematics or a related field







Experience with SAS/SQL/Excel\. Experience using Tableau desirable







Experience creating queries in SQL







Understanding of Commercial and Medicare ACOs strongly preferred







Understanding of health care operations and systems







Understanding of basic terminology related to clinical quality measures/HEDIS







Knowledge, Skills and Abilities







+ Knowledge of SQL and other software applications\.
