Description Position Summary
The Care Coordinator provides support to the new Radiology Consultation Service .RCS. in the Department of Radiology. Working under the supervision of a physician, coordinates care in RCS by responding to requests for consultations from referring physicians and from patients. Performs initial workup of the patient by collecting, organizing and summarizing pertinent clinical data and supporting materials such as physician notes, prior imaging studies, laboratory results, etc. Performs outreach and education and arranges for RCS participation in selected clinical services and inpatient units to assist in real.time delivery of care in multi.disciplinary settings.
Job Responsibilities
+ Works closely with radiologists and assists in tracking of critical results and imaging follow.up recommendations.

+ Responsible for quality monitoring and quality improvement activities of the Radiology Consultation Service.

+ Respond to consultation requests from referring physicians. Perform outreach and arrange for participation in selected clinical services or clinics and inpatient units to assist in real.time delivery of care in multi.disciplinary settings.

+ Collect, organize and summarize clinical data and supporting materials and presents to sub.specialty radiologist. Work closely with radiologist to develop imaging plan. Document and assist in communicating plan to referring physician and to the patient.

+ Schedule and coordinate consultation between patients and radiologists. Assist in scheduling complex imaging cases which require the presence of the Radiologist during exam.

+ Responsible for arranging with inpatient and clinic care teams Radiology Consultation Service participation in selected clinics, inpatient units, etc. and assists in real time delivery of care in these settings.
Education
+ Bachelor's Degree
Experience
3 – 5 years clinical experience as a Physician Assistant
Knowledge, Skills and Abilities
+ Excellent oral and written communication skills, ability to multi.task and work collaboratively with radiologists, referring physicians, technologists.

+ Ability to work efficiently, quickly, and carefully in a busy environment.

+ Professional and friendly manner with patients.

+ Evidence of leadership, advocacy, communication, education, and consultation.

+ Clinical understanding of IRB and manuscript writing.
Licenses and Certifications
+ Current New York State Physician Assistant License & Registration

+ BLS certification .issued by the American Heart Association.

+ Current ACLS or PALS .if applicable.

+ Infection Control Certificate

+ Current Narcotics DEA registration certificate
