Position Summary

Provides education to families and patients in inpatient/outpatient and community settings regarding the underlying disease process, proper identification of asthma triggers and symptoms, correct medication use and when to seek additional medical help\. The goal of this role is to avoid Emergency Department \(ED\) admissions and hospitalizations for individual patients and reduce overall ED/inpatient admissions\.

Job Responsibilities

+ Facilitates inpatient education sessions for hospitalized asthmatics admitted under the pulmonary service\.

+ Documents all educational sessions in patients electronic medical record\.

+ Works closely with providers and social workers when information regarding barriers to education are identified; will assist is some aspects of case management to coordinate care of selected high\-risk or severe asthma patients\.

+ Works with inpatient care coordinators to assess whether pre\-hospital discharge needs are met\.

+ Participates in monthly asthma program staff and clinical interdisciplinary meetings\.

+ Meets regularly with asthma community health worker\(s\) discuss patient cases\.

+ Is actively involved in community outreach events via telepresence or in person, when schedule permits\.


+ Bachelors of Science Degree in Nursing


Approximately 2 or more years of related work experience as a Registered Nurse\.

Experience in pediatric asthma education preferred\.

Enthusiasm to work within a medical team and with children and families\.

Multi or bi\-lingual \(Spanish/English preferred\)

Certified asthma educator is desired but not required\.

Knowledge, Skills and Abilities

+ Patience and a high capacity for empathy and perseverance\.

+ Excellent communication skills \(both verbal and written\)\.

+ Ability to exercise standards of professionalism, including appearance, presentation and demeanor\.

Licenses and Certifications

+ Current NY State Registered Nurse License & Registration\.

+ BLS Certification \(issued by the American Heart Association\)

+ Infection Control Certificate\.
